胆道系统疾病肝内外输送胆汁通道的疾病；肝外胆道系统包括储存胆汁的胆囊。胆道系统疾病种类很多，也较常见。胆石症为胆道系统所特有，感染、肿瘤也不少见。胆道是将肝细胞分泌的胆汁输送到肠道的惟一通路，胆道某一部位一旦发生疾病，即可导致胆汁引流不畅，对人体危害很大。一些胆道系统疾病如肝内结石、化脓性胆管炎、肿瘤等，治疗也较困难，可危及生命。胆道系统起源于肝内毛细胆管，沿着肝脏的解剖分叶，逐步汇合为各级肝内分支，至肝门部成为左、右肝管,分别引流左、右半肝,最后在肝外汇总为肝总管。胆囊经胆囊管与肝总管相通,自胆囊管进入肝总管以下,即成为胆总管。通常，将左右肝管以上的胆管称为肝内胆管，自肝总管以下称为肝外胆管。胆总管下端开口于十二指肠降部。胆囊有收缩功能，肝外胆管则无蠕动功能，仅仅是引流胆汁的通道。胆总管下端在十二指肠壁内斜行部分扩大成为法特氏壶腹，约70％左右的人，其胰腺导管也开口于壶腹，壶腹下端呈乳头状突出于十二指肠腔内，称为乳头。在壶腹部和乳头内均有括约肌围绕包括胰管开口在内的胆总管下端开口，称为胆总管下端括约肌或奥狄氏括约肌。正常人肝细胞每天分泌800ml左右的胆汁,平时奥狄氏括约肌处于收缩状态，使胆汁能够流入胆囊予以储存并浓缩。进食后十二指肠粘膜受到刺激，分泌胆囊收缩素，后者能使胆囊收缩,并使胆总管下端括约肌舒张,胆囊内的胆汁得以通过胆总管排出至十二指肠参与消化过程。概括起来，胆道系统在解剖和生理上的特点为：①作为管道系统，必须保持通畅，但又需根据生理需要来控制其排入十二指肠的节奏。②胆囊为胆道系统的憩室样结构，与胆管交通，是胆道系统的一部分，而又自成器官。③胆道系统与胰腺通常通过壶腹的共同通道而有密切的解剖和生理上的联系。④胆汁完全经胆管系统流入十二指肠，胆汁成分的变化对胆道系统造成直接的影响。⑤毛细胆管是肝实质的一部分，胆道系统和肝脏的异常，均相互影响。肿瘤、感染、梗阻、寄生虫病、先天性畸形如胆道团锁、胆总管囊肿等都可以在胆道系统发生。此外，胆道系统还有其特有的常见疾病，即胆石症。胆道的各种疾病常互为因果，例如感染可促使胆石形成，胆石又终将造成急性或慢性炎症。胆道的梗阻使胆汁淤滞，不但影响肝脏功能，而且常导致继发感染。胆石的慢性刺激有可能和癌的发生有关。由于局部解剖复杂，以及和肝脏的关系密切，胆系疾病的治疗较困难，且有一定的复发率和死亡率，所以胆系疾病一直是临床医生所面临的比较棘手的一个课题。近年来，诊断方法和治疗手段有了进展,B型超声、胆囊和胆道造影、γ照像、CT、逆行胰胆管造影等，使很多胆系疾病能够得到及时诊断。各种手术，包括一些新手术以及胆道镜取石、经皮经肝胆道穿刺引流等新技术的不断开展，对胆道系统疾病的疗效也有了显著提高。
